threatening is thoughts

about suicide which can be

pervasive because the
depression is so severe.

And the frequency of depression,
what are the statistics

very common illness.

And so it's estimated that
5% of the population at

issues that interfere in

treat depression.

One is stigma so that
people don't come forward

to get treatment.

The second is a lack of
understanding of the fact

that this is a medical
condition and requires

medical treatment.

And then I think the third
thing is that because the

people are hopeless you,
there's really not a lot

of motivation
to get better.

You really need an
advocate and a treatment

you forward and make sure

And so this is an illness
that by its very nature

to treat it.

What we do now for
treatment refractory

ketamine treatment,

vagus nerve stimulators,
electroconvulsive therapy

is our most effective
treatment for depression

and many of the patients
in the study have had these.

So people have not only
not responded to the usual

treatments they've had
some treatments such as

treatment, some more

experimental treatments
such as ketamine which is

anti-depressant and

haven't responded even
to those treatments.

generate neurotransmitters.

So we have a lot of
effective treatments for

moderate depression or
even severe depression

that's not treatment
refractory.

But what I think is
happening and this is, you know,

bit of work I've done so far,

is that if you're not generating
neurotransmitters,

blocking the reuptake to
increase their levels is not

going to be effective and that
may be part of the reason

we're seeing patients
that aren't

responding to multiple
medication trials.

So one of our forms of
treatment is to try to

increase a certain
neurochemical or

neurochemicals by blocking
how much it's taken back

that sends it out.

behavior and depression

to treatment.

And what had happened
is that we had tried

literally every medication
available, we'd even done

Lou Gehrig's disease

that's shown some promise
in anxiety and depression

and nothing happened.

has any control over.

You know structural
changes that make a brain

to engage in suicidal

biologic phenomenon.

And so when I see families
grieving and nothing -

that is for somebody who
has died, nothing was

done wrong, that this is
an illness, any more than

if somebody had a heart
attack you would say well

I could have prevented it.

It's a medical condition
and it's life threatening.

And I do think it's
crucial that, I'm not

threatening and that

should not seek treatment,

for things, but what I

want to say is that the
very severe end of the spectrum

this is a medical
problem and we need to

understand it better so
we can take care of it.

important point for

repeatedly hospitalized
so much that he couldn't

attend high school.

And when he started
responding to the

replacement therapy he was
able to graduate and he

went to college and
he's actually an

environmental scientist now and
he's in a healthy relationship

and he's happy.

There are days that depression
will still be present

but nothing like what
he came from.

And he talks about, you
know, trying to learn how,

how to live because he
had never planned to get

beyond the age of 20.

And so it was really a
fascinating change to watch.

And I've seen it with
other patients too.

We had another young woman
with the same disorder who

had a second medical
illness that was life

21:28.000 --> 21:31.000 position:17.5%,start line:79.33% size:62.5%
threatening and she would
allow that medical illness

21:31.000 --> 21:34.000 position:12.5%,start line:79.33% size:72.5%
to get out of control because
she didn't want to be alive.

21:34.000 --> 21:37.000 position:17.5%,start line:79.33% size:62.5%
And so she has also shown
significant improvement

21:37.000 --> 21:39.000 position:25%,start line:79.33% size:50%
and being able to go
to college which was

21:39.000 --> 21:43.000 position:17.5%,start line:79.33% size:62.5%
something I think she and
her family didn't expect.

So basically people are
able to have their life

back as a result
of this.

And it's been, it's been
really challenging because

we have patients coming
to us on referral for the

study and, you know, we
just with, with some

private funds, I'm
incredibly thankful for

the funding we have - the
Children's Hospital Foundation

and an anonymous donor
has given us funds,

and with some
private funds we've been

old man come to the study

who has been ill since he
was eight years old and he's

asking me like "if I have
one of these disorders."

And his profile fits
exactly what we see.

"You know I'd like to have
another I'd like to have

"10 years where I feel OK."

And it's heartbreaking
you know.

It really is devastating
when depression is

are available to people.
